Additionally, we ask that you complete the Traitify Assessment. We will need confirmation of your completion of this activity before your application can be completed. A link to the assessment will appear after submitting the application and an e-mail wil be sent to your account.

By submitting this application:

  • I certify the information given in this application is complete and accurate, and I understand that to make false or fraudulent statements within this application may result in denial of admission to the Student Support Services Program.
  • I fully understand that as part of my involvement in SSS, I must abide by the obligations and conditions set forth by the participant contract. (Applicants will have the opportunity upon acceptance into the program to accept or reject the terms and conditions of participation in the program.)
  • I authorize the Student Support Services program at Oklahoma State University to obtain, copy, review, and discuss records including:
High School Transcript   Standardized Test Scores  Final Grades and Transcripts
College Transcripts      Course Registration       Disability Documentation
Financial Aid Records Academic Progress Enrollment Information
  • I allow the Student Support Services program to take and use my photograph in marketing/recruitment materials, social media, and/or any appropriate manner.
  • The federal government requires that TRIO-SSS have on file documentation of taxable income, such as federal tax form, for all students admitted to the TRIO-SSS program; therefore, I will provide all applicable signed documentation of income.
  • Should any of this information I have given change prior to my acceptance in the program, I shall immediately notify the Director of the Student Support Services program.
